Breast implant sizing: beyond cubic centimetres

Cup size is not a unit and cc is not a personality. A Sydney sizing session should start with base width, tissue pinch and the clothes you actually wear.

Patients talk in cups. Manufacturers talk in cubic centimetres. Surgeons who are paying attention talk in base width, projection, tissue pinch and the distance from notch to nipple. The tragedy of a sizing session is when everyone agrees on a number and nobody agrees on a geometry.

This is the practical companion to profiles. If you only read one, read the measurements.

Why cup size is a marketing dialect

An Australian 12C in one brand is a rumour in another. Post-operative “I went from an A to a D” is a story, not a dataset. Bras are built for breasts that grew, not for devices with a fixed footprint. You may need a different cut, not a different alphabet letter, and you may need to be fitted after the six-month settle, not in the first fortnight when swelling is still writing advertising copy.

Ask the surgeon to speak in centimetres. Ask the fitter, later, to speak in bras. Do not make one do the other’s job.

The measurements that should be on paper

  • Base width of each breast, because you are probably not symmetrical
  • Sternal gap and how much cleavage you want versus how much your pecs will allow
  • Tissue pinch at the upper pole and the fold
  • Notch-to-nipple and nipple-to-fold, sitting and standing
  • Skin quality: thick, thin, stretch-marked, previously radiated, none of the above

If these numbers are not written down, the cc is a guess with a barcode.

Sizers, scans and the theatre of certainty

Rice in a stocking, silicone sizers in a sports bra, 3D photography that wraps a pretty picture around your torso — all of these can help you feel weight and see a silhouette. All of them lie a little. A sizer sits outside your tissue. A scan cannot promise how your pec will drape a dual-plane pocket. A rice bag has no cohesive gel.

Use the theatre. Then ask what the theatre cannot know. A good sizing session ends with a range, a preferred device, and a sentence about what happens if theatre measurements disagree with the consult.

The clothes test, done honestly

Bring the shirts you live in: the work blouse, the swimsuit, the jersey you wear to the cricket. Do not bring only the dress from a wedding you will attend once. Sydney life is repetitive. Size for the repetition.

If you are a runner, size with exercise in mind. If you hope to breastfeed, size with feeding in mind — not because size forbids feeding, but because a very large device on a small frame changes the mechanical story.

Asymmetry is the default

Most chests are sisters, not twins. Trying to force identical cc onto unequal footprints is how you get a lopsided neckline and a patient who thinks they failed at healing. Different volumes, or a small lift on one side, can be the adult plan. See tuberous breasts and congenital difference if the inequality is more than a polite cousin.

How to leave a sizing visit

You should be able to say: my base is X centimetres; we are looking at this profile; the volume is a range; the goal in clothes is this; the thing we are not trying to copy is that photograph. If you can only say a number, go back and ask for the rest of the sentence.

Cubic centimetres measure capacity. They do not measure taste, tissue or time. Put them last.

A worked example, without a prescription

Two people both “want 300 cc”. One has an 11.5 centimetre base and thin upper-pole pinch; 300 cc in a high profile may stand up like a scoops-apart pair. The other has a 13.5 centimetre base and leftover lower-pole tissue after feeding; 300 cc in a moderate profile may look like a continuation of a breast. The integer was never the point. The footprint was.

If a consult will not do this kind of arithmetic on your numbers, they are selling a box. Boxes belong in storerooms.

What “go bigger, you will regret small” is doing

It is a sales proverb with a grain of later-life truth and a pile of complications. Some people do wish they had more volume once swelling falls. Some people wish they could hide in a shirt. A proverb cannot see your sport, your job, or your skin. If a rooms uses it as the whole argument, they are managing their reviews, not your decade.

Ask instead: what do your patients with my frame say at two years, and how many of them you have revised for size. Numbers beat proverbs.

Sizing when you already have implants

You are not starting from a blank chest. The pocket, the capsule and the stretched skin are part of the tape measure. Going much larger can demand a new pocket and a new set of problems. Going smaller can demand a lift. Bring the old card. Bring the complaint in clothes, not only in the mirror.

Rice bags still lie, but they can tell you whether you are chasing a feeling of weight. Feelings of weight are allowed. They should still pass through measurements.

A closing note from this desk

Put the integer last. Put the base width, the pinch, the clothes and the decade first. If a rooms will not write the measurements down, they are asking you to trust a feeling they had while you were standing in their light. Feelings in their light are not portable.

Take the numbers to a second consult. If the second person uses different adjectives but the same centimetres, you are in adult territory. If they only offer a different integer, keep walking. Profiles will help you hear the ratio hiding inside the nickname.

The fitting after the settle

Do not buy a drawer of expensive bras in week two. Swelling is still a liar. At three to six months, go to a fitter who has seen implants and take the shirts you actually wear. You may land in a different letter than the one promised in the rooms. That is a clothing fact, not a broken contract.

If the fitter immediately says you need another operation, smile and leave. Fitters are not surgeons. Surgeons are not fitters. Let each do their verb. Then, if the clothes still fail and the measurements agree, you may have a revision sentence.

A last practical habit: write the chosen base width and projection in the same place as the cc. Future you, in a different suburb, will not remember which nickname meant what.

Questions readers send the desk

Why did two surgeons suggest different cc?
They may be using different base widths, profiles or brands. Compare measurements and projection, not the integer that happens to be on the box.
Do rice bags at home help?
They teach you something about weight on the chest. They do not teach you about pocket, muscle or how a cohesive gel sits when you lie down. Use them as a conversation starter, not a prescription.
